- Membership fees are due as outlined in your individual membership agreement
- Membership does not include the cost of laboratory work, prescriptions, or any services rendered by third-party providers
- Membership is non-transferable

6. Appointment and Cancellation Policy
We ask for a minimum of 48 hours' notice to cancel or reschedule any scheduled appointment. Late cancellations or no-shows may result in a cancellation fee as outlined in your membership agreement. We reserve the right to update this policy with reasonable advance notice.

Some consultations and follow-ups may be conducted via telehealth. By participating in telehealth services, you acknowledge that:

8. Prescription and Laboratory Services
Prescriptions, where applicable, are issued at the clinical discretion of Sarah Bettis, AGACNP-BC. Laboratory orders are facilitated through our practice but billed separately. We do not guarantee coverage by your insurance for any services, prescriptions, or laboratory work ordered through our practice.
